Note 2: As was rarely the case, the first page of the Intake Request was also in Katharina Schüddekopf’s file twice. Completed differently (different typewriter, slightly different responses).

Note 3: This page is in the file twice. The second version is completely different from the first. Same form, but filled out in different handwriting. Impossible to tell from (microfilmed) file copy if completed in red ink.
